According to the Ft Worth Star Telegram it was ISU, KSU, Tech and KSU who blocked the invites to the XII.

There was almost certainly it a vote. However, I'm pretty sure there was enough discussion so that everyone knew how a vote would go. Kinda like how they kept post postponing the Cal/Stan vote to the ACC until they knew how the official vote would go.
