Expectation bias by PAT25, 5342 must have been in a blind spot higher,to his left descending. Prob no ADSB on, no RA by 5342. ATC made a fatal mistake by giving the helo pilot benefit of the doubt. You can hear the nervous TFC updates, gotta take control there. Pray for survivors

I guess you've never flow into Reagan. One of the most congested airspaces in the world. You routinely see helicopters on approach flying the Potomac, circling the city, etc.cowboycwr said:

I haven't flown in and out of DCA a whole lot but I know the area. A lot of times the approach follows the bends in the river to avoid flying over the city. The main runway (01) is 7100' and is lined up so that at least the final approach/takeoff are over the longest straight stretches of the Potomac. However, since this was a smaller Bombardier CRJ, it was capable of landing on the shorter runway 33, which is only 5500' long or so. Too short for most full-sized commercial jets except when there is no alternative. But 33 is aligned at an angle to the river that requires coming in over Anacostia and crossing the Potomac almost perpendicularly. So any other commercial aircraft probably wouldn't have been in this approach.
If the copter was thinking the commercial traffic landing was all coming straight up the river ahead of him, he probably didn't notice or focus on one coming across the river at the same time as him.

None of that explains why there would be a helicopter flying directly in the path of a runway, imo.
To me that seems like a no brainer, no matter how congested the airspace is. Seems like that is a no go area at all times.
